Diabetes Management
Diabetes Classes: For patients with Type 2 diabetes. This five-hour class is taught in two sessions and teaches you about the disease and lifestyle changes for lifetime management of diabetes. It includes detailed discussion on meal planning, benefits of exercise and background in physiology, complications and medications.
TIME:
Mondays, 2 - 4:30 p.m.
Tuesdays, 9:30 a.m. - noon
Wednesdays 2 - 4:30 p.m.
FEE: $28 or co-payment (for CMG managed care) for each class session

Diabetes Counseling
Medical nutrition therapy for Type 1 diabetes and people having difficulty managing their Type 2 diabetes.

Gestational Diabetes
This two-and-a-half-hour group session is intended as the first consultation with a dietitian to give basic information about diabetes, nutrition recommendations, meal plans and home glucose monitoring instruction. All follow-up appointments are one-on-one to individualize care.
TIME: Wednesdays, 9:30 - noon
FEE: Co-payment for CMG managed care patients, $112 without insurance

Heart Smart Class
Four hours of instruction over two sessions, one week apart, covering dietary and lifestyle modifications along with a discussion of risk factors for heart disease and stroke.
TIME:
Tuesdays, 3:30 - 5:30 p.m.
Thursdays, 2 - 4 p.m.

Medical Nutrition Therapy
Individual sessions for many nutrition-related medical diagnoses including gastrointestinal, renal, children’s nutritional health, eating disorders and weight management. Individual sessions and follow-up also for people with heart disease or diabetes. This involves individual nutrition assessment for the medical disorder and nutrition care planning.
