The Patient Resource Center (PRC) of Camino Medical Group has helped to guide patients with many health care needs for more than a decade. The resource representatives assist patients with a variety of questions and provide valuable information about our doctors, services, locations, health insurance plans, wellness classes and patient education materials.
Patient Guide & Information
Authorization for Use or Discloser of Health Information (Acrobat file) -- form for releasing medical records. You must fill out a release of information form (Authorization for Use or Disclosure of Health Information) to request a copy of your medical record. This form is available at all CMG locations. The processing time is 10 working days from the receipt of the request with payment.
