What is a Hospitalist?
A hospitalist is an acute-care specialist physician who manages your care in the hospital. Usually trained in internal medicine, a hopsitalist has the skills and expertise needed to provide care specifically tailored to meet the unique needs of hospitalized patients and their families.

How Will a Hospitalist Improve Your Care?
Because a hospitalist has a special focus on patients in the hospital, he or she is more accessible to manage your inpatient care. During the week, the hospitalist is available every day between 7 a.m. and 11 p.m. and will see you more than once a day, if needed. A hospitalist can help you achieve a smooth, speedy recovery by following up on tests and adjusting your treatment throughout the day as necessary.
The hospitalist is also close-by in case of an emergency and is available to spend more time talking in-person with you and your family about your care. On weekends, hospitalists are accessible by phone 24 hours a day by calling 650-940-7000 or 408-739-6000.
What Role Will Your Primary Care Physician Have During Your Hospital Stay?
Your hospitalist knows that excellent communication is the key to providing quality care. He or she will be in close contact with your primary care physician when you are admitted to the hospital and again when you are discharged. The transition from your primary physician to the hospitalist and back again will be seamless and nearly transparent to you and your family.
If any major issue occurs during your hospitalization, the hospitalist will advise your primary care physician. In this way, your primary doctor is aware of any changes in your condition. Of course, he or she can also contact the hospitalist at anytime during your stay to learn about your progress.

When You Go Home
When you are discharged from the hospital, your primary care physician will resume management of your medical care. To make this transition as smooth as possible, your primary care doctor will have direct access to all medical records from your hospitalization. To continue your care with as little interruption as possible, the hospitalist will recommend a specific time when you should begin follow-up care with your primary physician.
