The Patient Resource Center of Camino Medical Group has helped to guide patients with their health care needs for more than a decade. The resource representatives assist patients with a variety of questions and provide valuable information about our doctors, services, locations, health insurance plans. Directories and health education brochures are available at the center. To reach a Patient Resource Center representative, please call 650-934-7080, or you can visit them in person at our Mountain View Center, 701 E. El Camino Real, Floor 2, Mountain View, Monday through Friday from 8:30 a.m. to 5:00 p.m. There are also representatives from medical records and the business office 8:00 a.m. to 5:00 p.m., Mondays through Firdays, located on the second floor of the Mountain View Center to help serve you.

Your Primary Care Physician

We believe it is in your best interest to have your own primary care physician to coordinate your health care. Having your own doctor means you will find it easier to schedule urgent appointments as well as routine examinations such as for school, camp, insurance or other special circumstances. Seeing the same doctor each time makes it easier to diagnose your conditions, coordinate and personalize your care, and follow your progress effectively. This will result in better care for you! Women have a choice of seeing their primary care physician for routine gynecological examinations or choosing an Ob/Gyn physician for annual exams. Advanced Access

Camino Medical Group believes our patients should be seen by their doctor with easy access and availability. Our vision and goal with the Advanced Access program is to supplement our trademark of high quality care with timely access to physicians; patients can schedule an appointment with their primary care physician within a few days, and often, the same day.

Registration

Your Medical Record Number

Accurate and complete medical records are essential to good medical care and your safety. Your own unique, permanent medical record is treated with professional confidence. Its purpose is to provide a valuable reference for all Camino Medical Group physicians you may consult. To assure proper personal identification, we ask that you use your medical record number in all communications with us.

We recommend that you pre-register you and your family before your first appointment. Please come to your physician's office approximately 30 minutes before your first appointment. In order to prevent confusion and ensure accuracy, we will need your full name, date of birth, social security number, driver's license number, address, employer, insurance information as well as the complete name, address and phone number of a friend/relative not living with the patient.


Please note that patients are required to show their insurance card and make their co-payment (if appropriate) at each visit.

Making Your Appointment

Patients are seen by appointment for all primary care and specialist physicians. To make an appointment, please call your doctor's office. Be sure to have your medical record number available when you call. Our physicians generally see patients between 8:30 a.m. and 5:00 p.m. weekdays. Some physicians may have extended or Saturday hours. Please call your doctor's office for details.

You will receive an automated or personal reminder phone call one or two days before your appointment at the primary telephone number you listed on your insurance form with us. If for any reason you are unable to keep an appointment, please cancel or reschedule as far in advance as possible. This will allow other patients the opportunity to be seen sooner.

Medical Advice / On-Call Service

Some medical problems require attention outside of regular office hours. Patients may reach an on-call physician 24 hours a day at 408-739-6000 or 650-934-7000. If your personal physician is not available when you call, another fully qualified colleague in the department will be available to help you.
